These Terms constitute a legally binding agreement between you and PureForty ("we," "us," or "our"). By using our website, completing a health intake, or purchasing services, you acknowledge that you have read, understood, and agree to be bound by these Terms and our Privacy Policy.
2. Description of Services
PureForty operates a telehealth platform that facilitates access to licensed healthcare providers and compounded prescription medications for weight loss treatment. Our services include:
- An online health intake form for patient assessment
- Asynchronous telehealth consultations with licensed clinicians through our clinical partner, OpenLoop Health
- Prescription issuance by licensed prescribers where clinically appropriate
- Medication fulfillment and shipping through accredited compounding pharmacies
- Ongoing care coordination and patient support
PureForty is a technology platform. We are not a pharmacy, medical practice, or clinical provider. All clinical services are provided by independent licensed clinicians operating through the OpenLoop Health network.
3. Medical Disclaimer
Compounded drug products are not FDA-approved finished drug products and have not been evaluated by the FDA for safety, effectiveness, or quality. PureForty's services are not a substitute for in-person medical care. Not all patients will be approved for treatment. Do not use our platform for emergency medical situations — call 911 or go to your nearest emergency room.
Nothing on this website constitutes medical advice. The information provided is for general informational purposes only. Individual results from GLP-1 medications vary. Always consult a qualified healthcare provider with questions about your specific medical condition.
4. Eligibility
To use PureForty's services, you must:
- Be at least 18 years of age
- Be a resident of the United States in a state where our services are available
- Complete the health intake form truthfully and accurately
- Not have a known contraindication to GLP-1 medications as determined by a clinician
Providing false or misleading information on your health intake is a violation of these Terms and may result in your account being terminated without refund. Accurate health information is essential for safe clinical assessment.
5. Pricing and Payment
Prices for all services are listed on the pricing page and are inclusive of medication, clinician review, and shipping. Prices are subject to change without notice — the price at the time of your order is the price you pay.
PureForty does not use subscriptions. Payment is due at the time of order. We do not charge automatically or ship medication without your explicit order. We accept Visa, Mastercard, American Express, and FSA/HSA cards.
6. Shipping and Delivery
Approved prescriptions are shipped via free expedited delivery, typically arriving within 2–5 business days of prescription issuance. Shipping is available to US states where our pharmacy partner is licensed to ship. Shipping times are estimates and are not guaranteed.
PureForty is not responsible for shipping delays caused by carrier disruptions, weather, or other factors outside our control. If your order is lost or damaged in transit, contact us at support@pureforty.com.
7. Refund Policy
If your health intake is not approved by a clinician, you will not be charged, or you will receive a full refund if payment was collected upfront. For all other refund circumstances, please see our Refund Policy.
8. Prohibited Use
You agree not to:
- Submit false, misleading, or fabricated health information on your intake form
- Resell or redistribute medication obtained through PureForty
- Share your prescription or medication with any other person
- Use the platform for any purpose other than obtaining treatment for yourself
- Attempt to circumvent clinical review or prescribing requirements
- Engage in any fraudulent, abusive, or illegal activity in connection with our services
Violations of these prohibitions may result in immediate account termination, forfeiture of any amounts paid, and reporting to appropriate legal or regulatory authorities.
9. Limitation of Liability
To the maximum extent permitted by applicable law, PureForty and its affiliates, partners, and service providers shall not be liable for any indirect, incidental, special, consequential, or punitive damages arising from your use of, or inability to use, our services.
Our total liability to you for any claim arising out of or relating to these Terms or our services shall not exceed the amount you paid to us in the 90 days preceding the claim.
Some jurisdictions do not allow limitations on liability for personal injury or certain other damages, so the above limitations may not apply to you.
10. Governing Law
These Terms shall be governed by and construed in accordance with the laws of the State of Delaware, without regard to its conflict of law principles. Any disputes arising under these Terms shall be resolved in the courts of competent jurisdiction in Delaware.
11. Changes to Terms
We reserve the right to update these Terms at any time. When we do, we will update the "Last Updated" date at the top of this page. For material changes, we will notify current patients via email. Your continued use of our services after a Terms update constitutes acceptance of the updated Terms.
