The intake is a structured health questionnaire — not a video call, not a phone appointment. You complete it at your pace, on your device, at any time of day.

The intake asks about your current weight and height, relevant medical history, current medications, and your weight loss goals. It typically takes 8–12 minutes.

All information is reviewed by a licensed clinician — not processed by an algorithm. Your intake is the basis for a medical assessment, not just a form submission.

02 Step Two

Clinician review
within 24 hours.

A licensed US physician or nurse practitioner reviews your complete health intake. This is an actual medical assessment — they check for contraindications, review your medication list, and determine whether a prescription is appropriate.

Not all applicants are approved. If your intake reveals a contraindication — such as a history of medullary thyroid carcinoma or an incompatible medication — you'll receive a clear explanation and a full refund.

If approved, your clinician selects the appropriate starting dose based on your intake. You may receive a message with additional questions or clarifications before a prescription is issued.

Clinician reviews your intake. No video call required in most states. The review is asynchronous — the clinician reads your intake at their next available time, typically within 24 hours of submission.

Follow-up questions. Your clinician may message you through the platform for additional information before prescribing. You'll receive a notification to respond.

Prescription issued or declined. If approved, your prescription is sent to the pharmacy same day. If declined, you receive a reason and a full refund.

GLP-1 medications are not appropriate for all patients. PureForty does not guarantee approval. If you are not approved, you will not be charged (or will receive a full refund) and will receive guidance on alternative options to discuss with your primary care provider.

a registered pharmacy.

Once your prescription is issued, it is sent electronically to our pharmacy partner — an accredited compounding pharmacy with applicable state and federal certifications.

Your medication is prepared, quality-checked, and packed in temperature-controlled packaging. All injection supplies are included. A detailed instruction sheet is included with each shipment.

Shipping is free, 2-day expedited delivery via a licensed pharmaceutical courier. You receive a tracking number when your order ships.

Pharmacy
Prescription received by pharmacy. The compounding pharmacy receives your prescription electronically and begins preparation. Compounding is done in batches with quality testing.
Day 3–4
Shipped
Order ships. Your medication ships via free 2-day expedited delivery. You receive a tracking number via email and in your account dashboard.
Day 5–7
Delivered
Medication arrives. Your medication arrives in discreet, temperature-controlled packaging. You're ready to begin after reviewing the instructions with your care team.

This is a general guide based on the typical titration schedule. Your clinician sets the actual dose and schedule — this is not a medical protocol. Individual experience varies.

Week 1

First Injection

You administer your first dose — typically the lowest available starting dose. Most people feel minimal effects in week 1. Some experience mild nausea or reduced appetite within the first few days. The goal this week is tolerability and getting comfortable with self-injection.

Week 2

Adjustment Phase

GI side effects, if they're going to occur, are often most noticeable in week 2. Nausea, constipation, or loose stools are the most common reports. These typically improve. Eating smaller, lower-fat meals and staying hydrated helps most people.

Week 3

Appetite Changes

Many patients begin to notice more consistent appetite reduction by week 3. Food noise — the persistent thoughts about food — tends to quiet for people who respond to GLP-1 medications. Side effects often start to ease at this stage.

Week 4

First Check-In

Your care team will check in around day 28. They'll ask about tolerability, side effects, and any questions you have. Based on your response, they'll determine whether to maintain the current dose or proceed with the titration schedule.

A note on expectations

Weight loss in month one varies widely. Some patients see meaningful changes early; others see modest results until they reach a higher dose. Neither outcome is unusual. GLP-1 medications are generally evaluated at 12 and 24 weeks — not at week 4. Please discuss your individual situation with your clinician before drawing conclusions about how the medication is working.

How refills
actually work.

There's no subscription. You decide when you want more medication. The process is quick, and your clinician is kept in the loop.

You receive a refill reminder

Approximately 1–2 weeks before your current supply runs out, your care team sends a secure message prompting you to consider a refill. This is a reminder, not an automatic charge.

Quick clinical check-in

Your clinician may ask a few brief questions about how treatment is going — tolerability, side effects, or any changes to your health since the last prescription. For most patients on a stable dose, this is a short async message exchange, not a new full intake. Occasionally a dose adjustment may be recommended.

Choose your supply length and pay

Once cleared by your clinician, you log into your account, select the supply length you want (1-, 3-, or 6-month), and complete payment. Nothing is charged until you explicitly confirm. You can also change medication or dosage at this point if your clinician recommends it.

Medication ships

Your refill order is sent to the pharmacy immediately after payment. Same process as your first order — prepared, packed, and shipped with free 2-day delivery. Tracking is available in your account.

Pausing or stopping

If you don't want a refill — for any reason — simply don't order one. There are no cancellation fees, no lock-in periods, and no questions asked. If you're considering stopping the medication, we recommend a conversation with your clinician first to discuss tapering vs. stopping abruptly and what to expect.

How to reach
your clinician.

Your care team is reachable through the platform. There are no phone queues, no on-hold music, and no automated responses — just secure messaging with real clinicians and support staff.

Secure Messaging

Send a message to your care team at any time through your account dashboard. All messages are encrypted and reviewed by clinical staff.

Typical response time: within 24 business hours. Urgent medical concerns should be directed to 911 or an emergency room.

Clinician Notes

After every review, your clinician leaves a note in your account with their assessment, dose recommendation, and any instructions. You can reference these at any time.

Notes are written by the clinician who reviewed your case, not templated auto-responses.

Support Team

For billing questions, shipping issues, or account help, the support team is reachable via the same secure messaging platform. Medical questions are routed to clinical staff.

Support hours: Monday–Friday, 9am–6pm ET. We aim to respond to all messages same business day.

In a medical emergency

PureForty is a telehealth platform — we are not an emergency care provider. If you experience a medical emergency, call 911 or go to your nearest emergency room immediately. Do not wait for a message response from your care team. For urgent but non-emergency symptoms — severe nausea, injection site reactions, unusual pain — send a message through the platform and mark it urgent.

Process questions,
plainly answered.

In most states, no. The intake and clinical review are asynchronous — meaning the clinician reviews your questionnaire on their schedule without a live call. A small number of states require a synchronous (live) telehealth visit before prescribing; if your state is one of them, you'll be notified and given options before you pay.
Most patients hear back within 24 hours of submitting their intake. In some cases — particularly if your clinician has follow-up questions — it may take up to 48 hours. We don't approve prescriptions automatically or without genuine clinical review, which takes time.
You'll receive a message from your clinician explaining why a prescription was not issued. You won't be charged — or if payment was collected upfront, you'll receive a full refund. The clinician's note may include a recommendation to speak with your primary care provider about next steps.
Yes. Dose adjustments are part of normal GLP-1 treatment and are handled at refill check-ins. If you want to switch from semaglutide to tirzepatide (or vice versa), message your care team — they'll assess whether a switch is appropriate and guide you through it. Medication changes require clinical review.
Compounded semaglutide and tirzepatide should be refrigerated (36–46°F / 2–8°C) until use. Once opened, they may remain at room temperature for a limited time — check your medication guide for specifics, as this varies by formulation. For travel, a standard medication cooler works well. TSA allows insulin syringes and injectable medications in carry-on bags with proper documentation. Your prescription documentation is available in your account.
Message your care team immediately with photos of the packaging and vial. We'll coordinate with the pharmacy on a replacement shipment. Do not use medication that appears damaged, discolored, or shows signs of temperature compromise. Your safety is the priority — no one will question your concern or make you fight for a replacement.
