A licensed US physician or nurse practitioner reviews your complete health intake. This is an actual medical assessment — they check for contraindications, review your medication list, and determine whether a prescription is appropriate.
Not all applicants are approved. If your intake reveals a contraindication — such as a history of medullary thyroid carcinoma or an incompatible medication — you'll receive a clear explanation and a full refund.
If approved, your clinician selects the appropriate starting dose based on your intake. You may receive a message with additional questions or clarifications before a prescription is issued.

Your first month,

This is a general guide based on the typical titration schedule. Your clinician sets the actual dose and schedule — this is not a medical protocol. Individual experience varies.
First Injection
You administer your first dose — typically the lowest available starting dose. Most people feel minimal effects in week 1. Some experience mild nausea or reduced appetite within the first few days. The goal this week is tolerability and getting comfortable with self-injection.
Adjustment Phase
GI side effects, if they're going to occur, are often most noticeable in week 2. Nausea, constipation, or loose stools are the most common reports. These typically improve. Eating smaller, lower-fat meals and staying hydrated helps most people.
Appetite Changes
Many patients begin to notice more consistent appetite reduction by week 3. Food noise — the persistent thoughts about food — tends to quiet for people who respond to GLP-1 medications. Side effects often start to ease at this stage.
First Check-In
Your care team will check in around day 28. They'll ask about tolerability, side effects, and any questions you have. Based on your response, they'll determine whether to maintain the current dose or proceed with the titration schedule.
A note on expectations
Weight loss in month one varies widely. Some patients see meaningful changes early; others see modest results until they reach a higher dose. Neither outcome is unusual. GLP-1 medications are generally evaluated at 12 and 24 weeks — not at week 4. Please discuss your individual situation with your clinician before drawing conclusions about how the medication is working.
How refills
actually work.
There's no subscription. You decide when you want more medication. The process is quick, and your clinician is kept in the loop.
You receive a refill reminder
Approximately 1–2 weeks before your current supply runs out, your care team sends a secure message prompting you to consider a refill. This is a reminder, not an automatic charge.
Quick clinical check-in
Your clinician may ask a few brief questions about how treatment is going — tolerability, side effects, or any changes to your health since the last prescription. For most patients on a stable dose, this is a short async message exchange, not a new full intake. Occasionally a dose adjustment may be recommended.
Choose your supply length and pay
Once cleared by your clinician, you log into your account, select the supply length you want (1-, 3-, or 6-month), and complete payment. Nothing is charged until you explicitly confirm. You can also change medication or dosage at this point if your clinician recommends it.
Medication ships
Your refill order is sent to the pharmacy immediately after payment. Same process as your first order — prepared, packed, and shipped with free 2-day delivery. Tracking is available in your account.
Pausing or stopping
If you don't want a refill — for any reason — simply don't order one. There are no cancellation fees, no lock-in periods, and no questions asked. If you're considering stopping the medication, we recommend a conversation with your clinician first to discuss tapering vs. stopping abruptly and what to expect.

Your care team is reachable through the platform. There are no phone queues, no on-hold music, and no automated responses — just secure messaging with real clinicians and support staff.

Send a message to your care team at any time through your account dashboard. All messages are encrypted and reviewed by clinical staff.
Typical response time: within 24 business hours. Urgent medical concerns should be directed to 911 or an emergency room.
Clinician Notes
After every review, your clinician leaves a note in your account with their assessment, dose recommendation, and any instructions. You can reference these at any time.
Notes are written by the clinician who reviewed your case, not templated auto-responses.
Support Team
For billing questions, shipping issues, or account help, the support team is reachable via the same secure messaging platform. Medical questions are routed to clinical staff.
Support hours: Monday–Friday, 9am–6pm ET. We aim to respond to all messages same business day.
